Model numbers 4337, LY504, LY704 or LY700. Sold through Baby City, Farmers, The Baby Factory, Smiths City, Repco and specialty stores from 2004 to 2009.
What to do
Stop using the change table accessory and phone 0800 628 000 to arrange the return of the product for modification.
