$100,000 for angiogram, arthroscopy, capsule endoscopy, colonoscopy, cystoscopy, gastroscopy, hysteroscopy, laparoscopy, myelogram, myocardial perfusion imaging, scintigraphy, PET, CT and MRI scans
Covered under surgical maximum, excludes routine dental treatments
$250 per treatment up to $500 for "non-specialist" surgery
